About Anthem Insurance Coverage

Anthem is a part of the Blue Cross Blue Shield company. It’s one of the major health insurers in the United States, and coverage is available in 14 states. It’s licensed by the Blue Cross and Blue Shield Association, and in California and Nevada, Anthem is now technically known as Elevance Health. Plans and coverage vary depending on the state you live in.

Four tiers of coverage are often available through Anthem, but not every plan is available in all areas. The plans from Anthem include:

  • Bronze: These plans usually have the lowest premiums to pay every month, but the out-of-pocket costs to receive care are the highest. A policyholder with a Bronze plan from Anthem will be responsible for around 40% of their costs. The deductible is also the highest with a Bronze Anthem plan so it’s a trade-off. 
  • Silver: A plan that’s in the Silver tier has a balance between the costs of the premiums each month and out-of-pocket responsibilities for care. You might expect to pay around 30% of the cost of your care if you have a Silver plan.
  • Gold: Plans in the Gold tier have higher monthly premiums than Silver, but that’s in exchange for lower out-of-pocket requirements. You will be responsible for around 20% of the costs of care.
  • Platinum: This high-level plan requires the most expensive monthly premiums but the most extensive coverage. A Platinum plan will usually cover 90% or more of the costs of treatment, and it has the lowest deductible, while a policyholder would be responsible for no more than 10%.

What Types of Rehab Will Anthem Cover?

There are inpatient and outpatient levels of care for addiction treatment. An inpatient program is residential, so you would live onsite for intensive treatment for a period of time. Your insurance may cover this stay but could limit the treatment you can receive.

Then, once you’re ready, you might begin an outpatient program, also usually covered by insurance. Someone with a milder addiction or substance use disorder might not go to inpatient rehab first. They could begin their treatment process in an outpatient setting.

Outpatient levels of care that Anthem could cover include:

  • Partial hospitalization programs: PHPs are the most intensive outpatient treatment level at most facilities, including Peninsula Health Center. During a PHP, also known as day treatment, much of your treatment is similar to inpatient rehab, but you can return home in the evenings.
  • Intensive outpatient programs: IOPs are somewhere between a PHP and a regular outpatient program. During an IOP, you might participate in group and individual counseling and holistic therapies.
  • Outpatient rehab: Offering flexibility and personal accountability, an outpatient program is a good fit for someone who’s ready to manage their recovery but needs some level of continued support. You return home in the evenings, and the required time commitment is less than PHPs and IOPs. 
  • Medication management: The Food and Drug Administration has approved several medicines for the treatment of alcohol and opioid use disorders. These medications, known as medication-assisted treatment (MAT), may be covered by Anthem insurance.

When someone has a mental health disorder that needs to be treated as part of their rehab program, known as dual diagnosis. The likelihood of a relapse is much greater without treating underlying mental health disorders.

As with substance use disorders, insurers often must provide mental and behavioral health services coverage. This can include counseling and therapy, such as cognitive-behavioral therapy. It can also include mental health medications.
